Overview (Baby Boy Nelson has returned to his mother's room)
Baby Boy Nelson has just finished breastfeeding. Dr Lopez, the pediatrician, examines the newborn and writes the following orders:
• Energix-B 10 mcg IM X 1 prior to discharge
• Discharge home with mother
• Follow up in office in 2 days
• Diet: Breastfeed every 2-3 hours on demand or Formula of choice 1-3 ounces every 3-4 hours
• Notify pediatrician if infant has:
o A temperature > 100.4
o Less than 6 wet diapers in 24 hours

O2 Sat = 99% (room air)
• Activity: Quiet, awake
• Fontanelles soft and flat
• Eyes symmetrical
• Nares patent
• Clavicles intact
• Heart sounds: Regular
• Lung sounds: Clear
• Bowel sounds: Active
• Extremities: Flexed, with good tone
• Pulses: 2+ and equal (brachial & femoral)
• Reflexes: Moro, suck, gag, rooting, palmer &
plantar grasp, Babinski, present
• No hip click noted
• Spine intact
The following tests/screenings are ordered on all infants prior to discharge:
• Critical Congenital Heart Disease (CCHD) Screening after 24 hours of age
• Transcutaneous Bilirubin and/or Total Bilirubin prior to discharge
• Hearing Screen
• Universal Metabolic Screening aka Metabolic Screening
