In their first chapter, Weick and Sutcliffe describe the practices characteristic of high reliability organizations: characteristics they see as increasingly important to all organizations, given the nature of today’s work environments. By creating what they call a “mindful infrastructure,” organizations can better respond to unexpected events; that is, “notice the unexpected in the making, halt it or contain it, and restore system functioning.” [1] They explain the five key operating principles which, indeed, make sense for organizations attempting to perform with high reliability in an “age of uncertainty.”
