PART A - Online Survey Creation and Facilitation

• You will create an online survey that will be completed by at least 30 of your fellow Master of Counselling students. All instructions will be geared solely towards the "Microsoft Forms" surveys, which is free with any Microsoft 365 account. You are welcome to use any online survey creation services that will allow for the criteria described in this assessment. However, be aware that most free online survey creators no longer allow you to download the data with their free account, you will need to get a paid account for that. If you don't have access to Microsoft Forms, Google Forms may be the second best option for a free account. The entire process for creating an account, creating a survey and sharing it with others in Microsoft Forms, is reviewed in Week 4.

• The title of your survey will be the research question that you created in Assessment 1. Your survey items should also come from the five survey questions you created in Assessment 1. Assessment 1 should be graded and returned to you by the end of Week 7. You are allowed to make adjustments to your Assessment 2 research question and survey items based off the feedback you received in your graded Assessment 1 essay, but there shouldn't be changes beyond that.*

*Note: If you feel the task requires you to make major changes beyond what the grader suggested or, if for some reason, your research question and survey items were not graded, you will need to send these to your lecturer at least 3-business days prior to the release of your survey. • Your title will need to be followed by the ‘Consent Text' which you can copy and paste from the text, exactly how it appears, in Appendix 1 (below). This ‘Consent Text' will need to be immediately followed by the ‘Student Number' question and this question should require an answer (see Appendix 1 image). If your survey questions require any brief instructions, you can include this after the ‘Student Number' question and these will not count towards your word count limit.

• After the ‘Student Number' question, add your five multiple choice questions. These questions can take any of the form of any of the Microsoft Forms question functions (Multiple Choice, Multiple Choice - Multiple Answers, Rating, Date, Ranking, Likert). You may not use questions that require a ‘Text' answer. You may, however, use the ‘Other' option in a multiple-choice question as the sixth answer choice which also includes a short text box. If one of your questions has more than 6 answer options, it will count as two of your five questions. If you

use a Likert question, each row counts as one survey question. You do not need to add any concluding language to your survey. Your title, five survey questions and their answer choices should not exceed a total of 200 words.

• On the HELP page, in Week 8, click into the 'Assessment 2-SURVEYS!' forum. Click on ‘Add discussion topic' and enter your survey title in the ‘Subject' field and the link to your survey in the ‘Message' field. Then click on ‘Post to forum.' (The Part A assignment is not complete until this is done).

PART B - Online Survey Participation

• Students are required to be survey respondents to, at least, 30 of their fellow students' surveys. If 30 surveys are not available to be completed, please return to the forum after Thursday of week 9 to see if there are any new surveys available. If there is still not the opportunity for you to complete 30 surveys, you will not be penalised. You will not be penalised if you accidentally complete more than 30 surveys but try to keep it at 30.

• On the HELP page, in Week 8, click into the 'Assessment 2-SURVEYS!' forum. You should see a number of ‘Discussions' listed. Each ‘Discussion,' is a survey if there is an indication that it is

‘FULL,' then that means the survey has already been completed by 30 people and you do not need to click on it. Otherwise, click on the ‘Discussion' and the link to the survey should be there. You may need to copy and paste this link into your browser. Please do not add comments to other peoples' discussion entries, regardless of whether it is positive or negative.

• Please ensure that you are fully considering each survey question and answering them openly and honestly. Do not answer your own survey. Every survey will require you to enter your student number, so you may want to copy and paste this in. If you have an issue or concern about a survey, please contact the lecturer. If you are uncomfortable answering a survey for any reason, you can always skip it and move on to the next one.

• Please monitor the number of responses to your own survey (e.g. turn on email notifications and/or subscribe to your ‘Discussion') and try to close the survey as soon as 30 people have completed it. Before closing the survey, check the data to ensure that there are 30 rows of data (e.g., the student numbers look legitimate and they are all unique, and all five questions were answered). Then add a comment to your discussion entry indicating that the survey is "FULL." Do not delete the ‘Discussion.'

• If you receive more than 30 responses to your survey, you will need to delete all data past the 30th entry. DO NOT delete any of the data from the first to 30th student respondents. If, for some reason, a row of data isn't complete (Student # and five answers), it does not count as one of your 30 responses. Contact your lecturer if you find yourself in need of more respondents.
