History of present illness: This patient, Eleanor Simmons, was hospitalized 10 days ago for management of CHF and urinary retention. She has a history of Type II DM, HTN, CVA, and Dementia. She was discharged to a nursing home with a Foley catheter. She has now returned to the hospital due to altered mental status, abdominal pain, weakness, chills, and nausea. She is in for R/O UTI. The Foley is now draining a moderate amount of cloudy foul-smelling urine with clumps of sediment.

1. How could the recent diagnosis of CHF and urinary retention have contributed to the current problems/diagnosis?
2. How does a history of Dementia and a CVA impact the nurses' ability to assess the patient and make an accurate interpretation?
