Mrs. R is a habitual absentee and remains absent without any intimation and proper sanction of leave. Her absentee records show that she has remained absent from her duties without proper sanction of leave for 96 days during the last one year. She was advised a number of times to improve on her attendance but despite verbal advises and assurances given to her, she has not shown any improvement in her attendance. Habitual absence on the part of an employee is a major misconduct under standing orders of the company. As an Industrial Relations Officer, of the company you have to issue her a charge sheet for habitual Absence. Please draft the charge sheet to be issued to the concerned employee.
