Question

Emmanuel is a 47-year-old Hispanic male who presents to the outpatient primary care office at the request of a follow-up by another provider in the office who only completes initial appointments. This follow-up was requested for secondary to concerns for substance abuse and depression. On initial screening at his last visit with the other provider in the office he noted on the assessment form that he drank 4 or more times per week and that his usual alcohol consumption involved three 40 oz beers on each occasion. He notes that on special occasions (typically once a month) he will drink upwards of 6 drinks in the context of social gatherings with friends or family. He notes that his mood has been increasingly sad /depressed since sustaining an injury that occurred while drinking. He endorses guilt over lost work secondary to his drinking. His has been married to his wife Selena for the past 25 years and has two adult daughter ages 18 and 21. Emmanuel reports that his wife has been increasingly concerned by his alcohol use as he often uses alcohol to self-medicate from the pain associated with his injury. His mood has been depressed and he reports not.
