You will need to complete and submit these sections of your Portfolio:
• Section 1: Identifying skills and skill development planning.
• Section 2: Assessor observations.
• Section 3: Reflective journals.
Information for the supervisor
Note to supervisor: When you see this icon, action is required on your part. Read the student's account of events and confirm their accuracy. The assessor will use your feedback to contribute to their assessment decision. You may like comment on whether the student has:
• followed your organisation's policies and procedures
• worked in a manner consistent with your organisation's values
• worked within the scope of their role
• sought guidance and feedback from you
• worked safely.
Please make sure the student is suitably supervised and that they have permission from everyone involved to engage with the client. This may include permission from the client directly or may require permission from their family member, carer or advocate.
Section 1: Identifying skills and skill development planning
In this section of your Portfolio you will work with three people to identify their strengths and goals and to develop a plan for skills development.
The first two people who you work with will be in simulations - you need to complete a separate copy of Template 1 for each person.
The third person who you work with will be in your workplace and you should complete Template 2 for this person. There is space at the end of each part of Template 2 for your supervisor to provide comments about your performance. Ensure that they have endorsed each section of Template 2 before you submit your Portfolio.
Developing development plans should align with the person's identified goals and organisational requirements.
Provide a brief summary of this person with details such as their age and background.

Part 1 - Identifying strengths and goals for skill development
Reflect on the work that you did with this person to identify an area for skill development. While you may have identified a number of areas for skill development during your work with this person, select one to focus on for the purposes of this assessment.
Describe the strategies that you used when working with this person to support them to express their goals and aspirations for skills development.
Did you involve the person's family and/or carer/s in this process? If so, how did you include them?
How did you support this person to identify and recognise their existing skills and strengths in relation to this goal?

Section 2: Assessor observations
In this section of your Portfolio your assessor will observe you as you support people with disability with ongoing skill development.
They will use Assessor observation checklist 1 for the first two people who you work with (simulations).
They will use Assessor observation checklist 2 for the third person you work with (who will be in the workplace).
Where appropriate, learners can also refer to Business Communication resources to strengthen professional communication skills required during workplace observations.
The following must be directly observed by your assessor. You must contribute to ongoing skills development, using a strengths-based approach, for at least three people with disability according to their individualised plan, two in simulation and one in the workplace, including:
• identifying existing skills, preferences and goals of the person and strategies and resources to employ to meet their goals
• using positive approaches and strategies that upholds the dignity and respects the rights of the person
• supporting the involvement of the person's carer or family
• providing feedback to the person
• monitoring strategies and adapting support to meet changing needs of the person, in consultation with the person and others.

Section 3: Reflective journals
In this section you will reflect on your work as you contribute to the ongoing skills development of each person. You will find questions to answer and examples of evidence to provide to support your entries.
Complete a copy of the Reflective journal for each person you work with.
There is space at the end of Journal 3 (for Person 3) so your supervisor can provide comments about your performance. Ensure that they have endorsed each section of Journal 3 before you submit your Portfolio.
Journal 1: Person 1 (simulation)
Part 1 - Barriers and difficulties
Reflect on a barrier or difficulty in implementing a skills development activity that the person discussed with you and answer the following questions.
Describe one barrier or difficulty in implementing a skills development activity that the person discussed with you.
How did you support the person to overcome the difficulty or barrier?
In your answer, write about the following:
• Who else you sought assistance from to address the difficulty or barrier.
• What adjustments (if any) you made to the strategies you had in place.
Part 2 - Monitoring and adapting developmental strategies
In Section 1 of your Portfolio, you identified a monitoring strategy for one skills development area. Reflect on the implementation of that monitoring strategy and answer the following questions.
Describe the strategies that you used to monitor the implementation of the skills development activities.
Describe the outcomes of your monitoring.
Include in your answer:
• How effective were the development activities?
• What was the person's level of engagement in the activities?
• What adjustments did you need to make to the activities as a result?
• How did you engage with the person, their family/carer/significant others and supervisor when monitoring strategies?
Part 3 - Complete your documentation
Now that you have worked with this person to respond to their needs and to identify appropriate services and supports, it's time to complete your documentation. This could include:
• records
• reports
• referrals.
Complete the following table and attach all documents to your Portfolio. Keep in mind that your documents must be:
• accurate
• complete
• objective
• written using terms that can be clearly understood
• consistent with your organisation's policies and procedures
• consistent with your legislative requirements
• include detailed, factual information
• consider the confidentiality of the person and their family (with due regard for the circumstances under which you can disclose confidential information)
• completed with consideration for privacy requirements
• completed professionally
• deidentified
• compliant with the person's right to access their records.
