"Mr A is in his 30s and lives in a low income country in sub-saharan Africa. His family believed he was sick, maybe with a mental illness, and they didn't know what to do. Mr A lived with his sister, but she felt she could no longer care for him, so she placed him in a privately-run residential centre in the city. He remained there for almost two years. For most of that time he was shackled at the ankles and locked inside. The staff gave him antipsychotic medication, but they did not seek his consent for this. They did not explain why they thought he needed it, or how it might affect him. If patients refused to take medication, or became aggressive, the staff beat them with a stick. Mr A and his family were not given any information about his diagnosis. Mr A did not have permission to leave the centre. Eventually his sister agreed that he could be allowed home."
