Question
Situation: I am calling about Mr. Ackah being admitted with urinary retention and is scheduled to undergo TURP surgery today. I am calling about his informed consent for surgery. I have checked his temperature, BP, and auscultated his heart, and everything is normal. I am concerned that the decision on the informed consent will take longer than the slotted OR time for the surgery today.
Background: Mr. Ackah is alert and oriented. He was admitted yesterday for urinary retention and is scheduled for a TURP surgery today. His only medical history is hypertension and is allergic to penicillin. His BP was 142/86 at shift hand-off. He has a foley catheter for drainage and output was normal. He is NPO, with intact skin and in no pain.
Assessment: The patient verbalized that he understood the surgery procedure, but as he was about to sign the consent form, his family entered the room and decided to further discuss the consent process. They had not resolved the issue after 30 minutes.
Recommendation: I recommend rescheduling the patient for surgery later to allow for the family to resolve the informed consent issue; I recommend moving up to later scheduled surgery. I recommend the physician talk with the family regarding the surgery procedure.

2. Identify appropriate focused assessment for the client.
3. Identify the cultural needs of the client and family and assess how well these needs were implemented.
4. Identify multi-disciplinary team members that should be included in the circle of care.
5. CNO standard of practice - review and summarize obtaining and giving, consent requirements.
6. CNO standard of practice - review and summarize conflict management focus on nurse-client conflict.
