Assignment:

Karen is an 87-year old female with metastatic breast cancer. Her cancer has spread throughout her body. She was treated with surgery (bilateral breast removal), chemotherapy and radiation over the past 2 years.

Code Status: Do not resuscitate. Spouse named as her healthcare surrogate decision-maker.

Karen's children do not get along. The son wants all measures to be taken in order to save his mother. Her daughter wants her to be comfortable and realizes she is at end of life. Karen's husband is stuck in between it all.

Karen's children fight in front of her and the son continues to fight for all cares to save her. He wants aggressive treatment.

As the primary RN for Karen, how would you help the family dynamics in this situation? What other disciplines may be able to assist?
