CASE STUDY
Maria is 28 years old and recently gave birth to her first child, Elizabeth. Maria's husband, Peter, has approached you, concerned that Maria stays in bed all day and seems disinterested in the baby.
Maria's mother Lina is from Italy and is currently staying with Maria and Peter to help with the baby. However, Lina is due to return to Italy in three weeks. Peter also took leave from his work but must return next week or he will jeopardize his job.
Peter knows that Maria had used drugs as a teenager and suffered from severe depression for about two years thereafter. She did not get on with her father's authoritarian ways and drugs were her way of rebelling. Maria's parents moved back to Italy five years ago claiming that their daughter's drug-taking had caused them to lose face among their friends in the Italian community.
Peter is concerned that the birth of their daughter, and Lina's visit, have raised unresolved emotional issues for Maria, and he is worried both for Maria's psychological well-being and the care of their daughter once Lina returns to Italy.
Maria is not very enthusiastic to discuss her issues but knows that something is not OK so she has agreed to attend counselling with Peter. Maria has told you that she finds it difficult to adjust to life with a newborn child, is always tired, lacking time to look after herself and is worried about managing the responsibilities once Maria's mother, Lina support returns to Italy.
Given that Maria is time-poor and fatigued, you as the counsellor are concerned about given Maria homework, or things to do that may cause her to retreat further into her depressive moods. You decide to apply narrative therapy which allows a person to tell their story by focusing on positive emotions and interpretations which is an important component of depression treatment.
During the second session, Maria said she felt the counselling was beneficial and has started to open up about her unsolved issues with her father which may have had a negative impact on her building an attachment relationship with Elizabeth. You suggest to Maria some ideas and strategies to help build her confidence and manage her time, including websites in case she needs support outside of counselling.
You also work with Maria using the "Map of the Position Statement" activity (narrative therapy) to help unpack the suppressed emotions and thoughts. You discuss with Maria the following questions:
1. Can Maria give the problem a name or express how it feels for her
2. How does the problem affect her life in areas such as looking after the baby, relationship etc
3. What thoughts or emotions arise when Maria thinks about her past issues such as drug use, depression etc.
In the third session, Maria stated her parenting skills have improved and she was feeling better about herself. The relationship with Peter was also improved and Maria was starting to build a bond with her baby, Elizabeth.
