Use each psychological model only once. Part 1: John is a 17-year-old cisgender male, who has been drinking heavily since he was 12 years old. He drinks almost every day and when particularly anxious, he drinks until he passes out. His father and grandfather were diagnosed with alcohol use disorders. His father died of liver disease when John was 14 years old. His mother and older brother do not drink alcohol and have always told John that he is the black sheep of the family and destined to be like his father.

Additional information: John's mother and father did not have a good marriage. There was a great deal of fighting in the home. Even though his mother does not drink, she does use prescription medications (and often more than prescribed) to manage anxiety. John lives in a neighborhood where alcohol use is the norm. He and his friends have access to unlocked liquor cabinets and there is always a home where they can gather unsupervised.

Additional information: John was a particularly fussy baby who did not like to be held, had trouble soothing himself, and slept poorly. His preschool teachers said he had trouble playing by himself and struggled to play well with others. School has always been difficult for him. He remained motivated to please teachers and his family until the environment felt less supportive. He started feeling overwhelmed, had low energy, and would be hard on himself.
