Alejandro, a 63-year-old male, was admitted overnight via the Emergency Department after an intentional polypharmacy overdose that included antidepressants and paracetamol. Brought in by SAAS after family member reports finding him unresponsive, slumped in lounge chair, amount of medication taken is unknown.
He was admitted to the ward for observations and medical management that includes monitoring paracetamol blood levels, and a NAC (N-Acetylcysteine) infusion was commenced. Alejandro regrets that he did not die. He reports that he experiences persistent sadness, hopelessness, and a loss of interest in activities.
Background History:
Alejandro was diagnosed with Major Depressive Disorder by his General Practitioner some 8 months ago and is prescribed anti-depressant medication at this time. (Selective Serotonin Re-uptake Inhibitor).
Previous Medical History:
Heart Attack 12 months ago resulting in cardiovascular disease.
Symptoms
Hypersomnia, fatigue, decreased appetite, and recurrent thoughts of self-harm.
Loss of interest in activities he once enjoyed including cooking meals, and more recently neglects self-care. Describes his mind as foggy, making it difficult to think or make decisions.
Social History:
Currently unemployed and receives Centrelink benefits.
Estranged from his wife and children, most of his family living overseas in the Philippines.
Moved to Australia five years ago with limited language proficiency and limited friends.
Support System:
His niece, Divina, who provides emotional support, arranges appointments, encourages him to attend therapy sessions, and supports him with the administration of his prescribed medications.
