Overview:
In this homework, you will conduct a windshield survey of a selected community and develop a targeted intervention proposal based on your observations and data collection. You will focus on a specific public health issue from the assigned topics and propose a one-time, four-hour health event intervention to address the identified need. This intervention will be held at an appropriate local venue, such as a grocery store, drug store, or park, and should align with Tier 1 Community/Public Health Nurse competencies.

I. The intervention must be focused on the community/ population (not individual patient/family) within your own community (community used for the Windshield Survey paper). This must be within the scope of the Tier 1 staff level public health/ community health nurse. (Note: you cannot propose building facilities, developing a large program, expanding mental health care, or purchasing a mobile health van). Please refer to the QUAD COUNCIL Competencies for Community/ Public Health Nurses.
II. The baccalaureate degree in nursing (BSN) is the educational preparation for entry level C/PHN practice. The varied courses in a BSN Program provides the grounding in order to understand how personal, social, policy, economic, work, and environmental determinants affect health status of individuals, communities, and populations. BSN education prepares PHNs both didactically and clinically. The paper should be 6-8 pages excluding title page and references.
III. The following topics must be covered in the paper. You are a CHN/PHN in a local health department (your community from Windshield Survey). The intervention is a one-time four hour event held at a location (grocery/drug store, park or appropriate to the topic) and is not repeated.
IV. YOU DO NOT ACTUALLY DO THE EVENT- you are not covered by FSW insurance. This is a proposal.
Task
I. Introduction:
1. Clearly identify the health need based on your windshield survey and provide a summary of relevant data supporting this need.
2. Discuss the relevance of the identified issue to public health nursing practice.
3. Use data collected during your survey, along with additional research, to justify the need for your proposed intervention.
II. Proposed Intervention:
1. Describe the intervention in detail, including the location and format of the one-time, four-hour event.
2. Provide evidence from research and literature to support the effectiveness of the intervention.
3. Identify the level(s) of prevention your intervention addresses (primary, secondary, or tertiary).
4. Develop a specific objective for the event that will be reflected in your measurable goal.
III. Resources Required:
1. Identify key stakeholders and describe how you would involve them in the intervention.
2. Provide a breakdown of the estimated financial costs, not exceeding $500, and explain how the budget will be utilized.
3. Discuss how you will obtain other necessary resources, such as facilities, locations, personnel, and equipment.
IV. Implementation Plan and Timeline:
1. Outline a detailed plan for implementing the intervention, including specific steps and a timeline leading up to and during the event.
V. Evaluation Method:
1. Develop a SMART goal that reflects the objective of your intervention.
2. Describe the specific evaluation methods you will use to assess the effectiveness of the intervention, including measurable criteria.
