Case Study: Priya is a first-year nursing student on clinical placement for HNN112. During her shift, her preceptor, who is busy with another patient, instructs her to take and document the vital signs of Mr. Thompson, a 72- year-old recovering from colorectal surgery, which are overdue. Lacking confidence in her ability to accurately measure vital signs on her own, Priya feels uncertain about performing the task unsupervised. However, she does not want to appear useless. Feeling overwhelmed, she decides to document vital signs similar to those previously recorded, without actually measuring them. Later in the shift, Mr. Thompson develops postoperative complications, including tachycardia, hypotension, and fever. When the healthcare team reviews his records, they notice discrepancies between the documented and actual vital signs. Upon questioning, Priya admits she did not assess the vital signs and had fabricated the data instead.
