Hematologic Assessment
The patient is a 45-year-old married woman. Her father was a miner, leading an itinerant life as he worked in uranium mines. She remembers playing with other children on slag heaps from the mines. She recalls frequent episodes of illness during childhood and attributes this to being chronically undernourished. The family was often cold during the winter months when there was no money to buy warm clothing or fuel for heat.
Over the past 6 months, the patient has had episodes of epistaxis. She reports prolonged bleeding after having her teeth cleaned by a dental hygienist. She has also noticed that she bruises easily, and there are multiple ecchymotic areas on her legs and arms. She was referred by her primary care provider for further evaluation. During the admission history, she reports that she tires easily and often has little energy. Her husband and children are worried about her.
1. What additional assessment data should the nurse elicit from the patient at this time?
2. As part of the diagnostic workup, the patient is scheduled to have blood work drawn for CBC, PT, PTT, INR, and TIBC. Discuss these laboratory studies to include what they measure.
3. The results of the laboratory tests and bone scan indicate that the patient has depressed bone marrow function. The hematologist discusses plans to perform a bone marrow aspiration with her. What is included in the preprocedure care and patient teaching for this patient?
