Case Study

Task description: You are required to demonstrate your clinical reasoning related to the assessment and treatment of a patient through two distinct phases of care: the acute phase and the chronic phase.

Read the case study with the associated medication charts and then address the following tasks from the perspective of the acute and chronic phase of the patient's condition.

1. For each phase: Identify one (1) priority problem and justify your choice by linking to the relevant assessment data and pathophysiology.

2. For each phase: Identify one (1) other nursing assessment you feel would be necessary and provide a rationale for your choice.

3. For each phase: Identify two (2) interventions to address your priority problems and provide a rationale for the choice of each intervention

N.B. If your chosen interventions are collaborative with members of the multidisciplinary team, then you must focus on the requirements of the nursing role in that intervention.

4. For each phase: Demonstrate how you would evaluate the effectiveness of your chosen interventions.
