1) Irving Janis, a former research psychologist at Yale University and professor at University of California, Berkeley, created the 8 main symptoms of groupthink (p. 292). The main three that I believe apply to this are collective rationalization, illusion of invulnerability, and stereotyped views of opposition. Collective rationalization and illusion of invulnerability because they had already gotten away with flights below 53°F previously, and they were highly optimistic that they would be just fine again this time. Excessive stereotyping because I believe people that had final say in the launch doubted the engineers that designed the O-rings.
