Case study: Patient Error Analysis Assessment Instructions

This assessment of 1000 words (+/- 10%) requires you to apply concepts from the Mind Map (developed in NURS3002 Assessment 1A) in relation to the Vincristine case scenario:

Overview

Conduct a comprehensive analysis of one contributing factor (identified by you from the video) in relation to the patient safety error. You will examine how a specific component led to an adverse event and support your analysis with current literature.

Assessment Requirements

Part 1: Component Contribution Analysis (100 words)

Question: How did the selected component contribute to the patient error? Instructions:

Identify and clearly define the specific component you have chosen to analyse (e.g., communication breakdown, patient involvement, team hierarchy, medication process failure, staffing issues, distraction, interruption, environmental factors, etc.)

Provide an explanation of the sequence of events showing how this component directly contributed to the error

Link to specific examples from the video case study

Part 2: Causal Analysis (500 words)

Question: Why did the selected component cause the error? Instructions:

Conduct an analysis examining the underlying factors that allowed your selected component to contribute to the error

Consider multiple levels of causation:

Individual factors (knowledge, skills, fatigue, etc.)

Team factors (communication, supervision, etc.)

Organisational factors (policies, culture, resources, etc.)

System factors (design, processes, etc.)

Explain the mechanisms by which these underlying factors created conditions for error

Avoid focusing solely on individual blame as you need to demonstrate understanding of systems thinking

Consider both active failures (immediate unsafe acts) and latent conditions (underlying system weaknesses)

Part 3: Literature Review and Evidence (400 words)

Question: What does the literature say in relation to the cause of the error? Instructions:

Conduct a focused literature review on your selected component and its relationship to patient errors

Include a minimum of 5 peer-reviewed sources published within the last 10 years

Attach your search strategy from the Nursing databases you have used as an appendix in your document (see template)

An effective literature review should include at least 3 relevant databases. Students are advised to show a literature search that includes at least ONE of the following databases in their literature search: CINAHL, Pubmed, Medline

Other relevant healthcare databases to consider using include: ProQuest, Emcare, EMBASE, Cochrane Library and Joanna Briggs Institute for Evidence Based Practice.

Scopus, Web of Science and Google Scholar are general databases that would also be suitable alongside the other mentioned databases above. Note: A general Google search or just general search on the Curtin Library site would not be considered a suitable search of the literature or databases for a literature review, or to include as evidence in the Appendix of your assessment.

Your marker should be able to see where any papers cited in your literature review paper have been retrieved from in your search strategy

Synthesise findings from these studies, and compare and contrast different perspectives or findings in the literature

Identify any gaps in current knowledge or conflicting evidence

Relate the literature findings back to your specific case analysis

Key areas to explore in literature:

Frequency and prevalence of your selected component in healthcare errors

Risk factors that increase the likelihood of this type of error

Cover page or contents page not required for this assessment

Use scholarly articles about the topic. Students may also refer to relevant learning materials utilised in the unit learning modules where applicable

Word count DOES NOT include the end reference list or Appendix (search strategy)

Word counts DOES include everything else within your paper: content, in-text citations, headings, tables and quotes

Citations within text and end reference list to follow APA 7th Edition format

Recommend using at least 10 references throughout the paper, published within the last 10 years (2016-2026)

Indent the start part of each paragraph as per APA Publication style - see guidelines for formatting

Include the word count at the end of your paper
