Jane Smith, is a young child (DOB 1/01/2018; Address 16 That Street, This town, 1234) running around the foyer as her mother waits for her scheduled appointment with one of your supervisors.
As Jane plays around, her foot gets caught on the cleaning bucket that had been left half under one of the chairs. She stumbles and falls heavily on her left side, impacting her elbow on the tiled floor.
Hearing her cries, you run over to the child who says she has a sore elbow and that her hip and thigh hurt.
Mary Smith, Jane's mother, witnessed the fall and attends to her first, comforting and helping Jane stable her elbow still.
While Jane's mother comforts Jane, you go and get the first aid kit and return to Jane to apply basic first aid. As Jane's mother has helped Jane steady the arm at a right angle positioned across the front of Jane's body, you secure a sling (from the first aid box) to support and brace the arm/elbow still. Jane expresses that she feels her hip and thigh don't hurt so much anymore. Mary has a look and besides the area being a little red it seems ok.
As Jane settles and there are no other signs of injury, Mary requests that no ambulance be called and that she will take Jane to the nearest doctor to get a referral for an X-ray for Jane's elbow.
After Mary and Jane leave you ensure that the cleaning bucket is removed from the area and stored safely away. You then notify your workplace supervisor, Vivian. Vivian then asks you to complete the workplace incident and injury report
