Let's imagine a fire department that had a big incident where things went wrong. They had problems with communication and didn't have the right equipment, which caused delays in putting out the fire. Because of this major failure, they decided to make some changes in how they do things. Now, after every operation, they have meetings to talk about what happened and figure out how to do better next time. They also bought better communication systems and gave the firefighters more training so they can work together better and respond faster. These changes would help firefighters learn from their mistakes and get better at putting out fires.
