Case Conference / Service Plan Assignment
Part I - Research
1. Research the Internet / newspapers, text books, class notes or use a personal case study that you are familiar with regarding a child / youth who has an exceptionality. Search for a case that will allow you to respond to all the required information below. This will become your case study assignment.
2. Attach a copy of this case study to your assignment.
3. Research factual information regarding the exceptionality identified in your case and prepare a FACT SHEET that highlights 20 key points that will help you better understand this child and his / her exceptionality.
Part II - Case Management / Wraparound / Person-
Centered Worksheet
Assume that you are the (SSW / ECE) worker responsible for the child or youth identified in your case. You are asked to assess the child or youth in this situation and put together a service plan (using the wraparound process / case management / person-centered approach) identified below. If you are using an authentic person, please ensure confidentiality. This service plan is to be worked out using YOUR COMMUNITY.
I. Identify and describe the community you are in. (Community Profile)
2. Client / Family Identification:
- Summarize the client / family situation.
- Identify and prioritize the client's concerns as he / she sees it.
Briefly discuss each microculture (religion, language, gender, race, ethnicity, age, geography, income) as it applies to this client / family.
- Briefly discuss each family function (affection, spirituality, economics, daily care, socialization, recreation, education, self-esteem) as it applies to this client / family.
3. Stabilize the situation:
- Explain how YOU would stabilize the situation. - What resources would you inform the client / family of at this time?
4. Reframe the situation:
- Provide a statement that would reframe this negative situation to a positive situation.
5. Introduction to wraparound / case management / client-centered approach
- Discuss how you would orient the child / family to the process.
- What would you say to the client / family to encourage them to accept your offer to assist them?
6. Assessments
- Perform a needs assessment. Identify the needs of the client and of each family member.
- Perform a strengths assessment. Identify 5 strength
questions for the client and of each family member.
- Based on the above, identify, prioritize and discuss the life
domains that need to be addressed.
- Based on the above, brainstorm and identify all the informal and formal supports who could possibly participate in this wraparound.
7. Plan of Care (Service Plan)
- Based on the needs and strengths of the client / each family member, choose the top three (3) needs / life domains and develop SMART goals for each.
- Using the (example) chart below, complete the above for each of the three goals.

Goal #1 Tommy will Team to exhibit appropriate social interactions with his peers by the end of this month. (IGO I/2016)

Time Frame - The following steps will begin on or about September 26, 2016 and be Dates of Service followed to completion on or about October 31, 2016.
8. Plan Implementation & Transition
- Identify and discuss the following:
o How often you would hold wraparound / case management meetings.
o Where the meetings would take place (natural environment).
o How you (the worker / facilitator) would maintain
cohesiveness / trust / and open communication.
o How you would monitor the client / family members / formal members.
o How you would prepare the client / family members for termination.
o How you would "follow-up" with the client / family once the goal has been reached (and relationship is ended.)
9. Checklist ... identify and briefly discuss where in your wraparound / case management / person-centred process you have addressed the following applications in your service plan:

- A family-driven, family-strengths-focussed process - An individual approach

Part III - Summary Thoughts and Reflection
1. Meet with a fellow classmate and share with him / her your goals and intervention team. Explain to him / her
your reasoning for selecting the goals / intervention team. Ask your classmate for constructive feedback regarding your plan. Document this feedback (along with the name of your classmate.)
2. Analyze the feedback you received from your classmate.
a. Using your reflective skills, identify and discuss five (5) strengths that you possessed in working through this case study.
b. Using your reflective skills, identify and discuss three (3) weaknesses that you need to improve upon (pertaining to your ability in dealing with this case study.)
c. In relation to the above (c), identify three (SMART) goals, identifying how you will improve upon your weaknesses.
