There is an interesting article connecting cause and hazardous waste and effect analysis. The authors have used Effect Analysis (FMEA) and Failure Mode to successfully enhance disposal of hazardous waste in a hospital. They found out that a lot of infectious waste was not disposed correctly by. They show in a very clear table what the failure mode is, what the beginning of the failure is and what are the consequences. Once understood the causes and consequences of these actions, they were able to implement and draft an improvement plan through labeling and awareness for example. Their study show notable improvements in all categories. Therefore the FMEA is a simple method yet effective to improve quality in many sectors. It simply reminds us that sometimes quality is about common sense :-) What are your thoughts on that?)
