In patients who are not being considered for aortic valve replacement digoxine and diuretics can give symptom relief. In decompensated heart failure ACE inhibitors may be tried cautiously. Sinus rhythm should be maintained with amiodarone. Beta blockers or other drugs with negative inotropic effect should be used very cautiously or not at all. Nitrates may be given cautiously. Patients with moderate to severe aortic stenosis, even when asymptomatic, should be advised against any kind of vigorous exercise or physical activity.
