i have predefined template which contains all my user details and there order details

We have to create a login where a user uploads data.

Mobile  Name    C/o         Door No               Village   Mandal Postoffice           Address               District  State     Pincode                Landmark Tracking No   Date      Note      Note1

a group of users log into their individual ID and update record

a screen where agent will enter mobile no and enter some data in box and that may be updated in database

Detail explanation can be done.

Simple reports are required.
