i neeed an CGI Script that send mass emails

i need this input fields

Sender Name:
Sender Email:
Subject:

Message: choose (HTML Or text ) , by default HTML

Emails

and send button

the script should print the progress of sending
for example:

1- . Sent
2. , sent
3. etc.. sent

with grey background, Logo text: " CGI Mailer "

the script should use CGI Functions to send email

and there should be an option to use smtp as alternative with this fields

Host:
Username:
Password:
Port:


script should be only one file and the options should be in the top of page
