Accident report forms : Accident report forms are to be used in case of serious accidents. A properly completed accident report form should contain the following information:
(1) The date, time and location of the accident
(2) The name, address and age of the casualty
(3) A description of the accident
(4) The names of any witnesses
(5) Details of any injuries suffered or suspected
(6) A description of the first aid treatment given
(7) The name of the person who gave the first aid treatment
