Problem

Write a psych evaluation, chief complaint, HPI, psychosocial, Assessment and formulation, and plan for a 15 years old who came in for a psychiatric evaluation. Not on any psychiatric medications. He said he struggles with sleep problems, depressed and therapy has helped but he need additional help. He said he is depressed when a friend is having a stressful time ad he tries to take off some of that stress and it makes him feeling worse at the end. He said his sleep is the big problem because he does not get the best sleep. He said he struggles with falling asleep. He said at night, he usually put on some music and set it aside.
