In about two or three paragraphs, include information about the name of the aggregate, its geographical location and size, its population, and a brief history.

You can choose an entire town or a small community, or even a neighborhood in a large city, but clearly describe your aggregate.

Explain, giving at least two reasons, why you selected this particular aggregate for your Capstone project.

This week, you learned how Nurse Smith collected relevant demographic and statistical information about the aggregate she was assigned to care for.

You can use her experience to help you with your Capstone project. While working on your project, document your findings along with details of those professionals who assisted you.

• An aggregate is a collection of individuals who have one or more personal or environmental characteristics in common.

• Community-based nursing care involves setting up a specific practice. Care is provided for clients and families where they live, work, and attend school.

• Population-focused practice refers to an environment where problems are defined by assessments or diagnoses and solutions or interventions are implemented for a defined population or subpopulation.

• Epidemiologists study the distribution and determinants of health-related states or events in specific populations and then apply this study to control health problems.

• Applied epidemiology assists healthcare professionals to focus their problem-solving efforts on the emergent issues.

The Capstone project is a multipart project that spans ten weeks. The goal of the Capstone project is to select an aggregate (community or group of people), assess its health needs by conducting a risk assessment, develop a care plan for the aggregate and describe how you would implement the plan, and evaluate its effectiveness. Each week, you will complete one phase of the project, building upon the work completed in the previous weeks. Your completed Capstone project is due in Week 10.

You will be required to select an aggregate (population) to work with for this class; however, you will not be able to select a client population from your place of employment. You may choose your community, county, city, or another group or organization; e.g. a senior citizen's center, a school or a particular grade level in a school, a faith based group, a support group, or a group from your local public health department.
