HPI: U.O is a 28-year-old female who comes to the clinic complaining of a depressed mood for the past 6 months. U.O reported having a feeling of sadness, hopelessness, and loss of interest in her normal activities. She reports having disrupted sleep and loss of appetite. She reports fatigue and that her energy has decreased, which has made it difficult to carry out her daily tasks. The client denies suicidal ideation and also denies having other psychiatric symptoms. The client reports that her personal and professional life has been affected by the worsening symptoms. She reports no significant history of illness. She has no significant medical history and no psychiatric history.
