Case - Andrew is a 26-year-old Hispanic male who lives at home with his mother and older sister. He works part-time and attends a local community college part time as well. He has been in and out of school and has not yet graduated with his college degree. At one point, he was disciplined for smoking marijuana on campus and was put on academic probation. He has held a few part-time jobs, but usually is fired for poor performance after a few months. Currently, he works part-time at a local coffee shop. Andrew has three brothers and one sister. One brother died as a child in a tragic drowning accident. Andrew is estranged from his other brothers, but is in contact with his sister. While she often looks and acts depressed, she claims she is not, nor has she sought treatment of any kind. Andrew's mother however, has been placed on antidepressants by her Primary Care Physician at various times. Andrew recently sought counseling at the urging of his girlfriend. Andrew states that he is fine and does not need help. With much prompting, he admits to drinking alcohol and smoking marijuana. After a few sessions together, you begin to think that Andrew has been battling Major Depression for the past 10 years and that he has been drinking to self-medicate. His drinking has increased lately, and Andrew even boasts about how much liquor he can hold without getting intoxicated. Eventually, Andrew admits that he has tried in the past to stop drinking and smoking marijuana but his attempts have been unsuccessful. Now, he is at risk to lose his job again, as he keeps missing work.
