Question: Soap note about this A 40-year man presented to me with the complaint of acute central chest pain for 12 hours, vital signs HR 120 bpm, T 97 F, BP 140/90 satO2 89, H 6.4 feet W: 78kg, he describes as sharp, compressing pain, predominantly localized to the central portion of the chest and radiate to left arm. His chest pain worsens when he breathes deeply or lie down in bed and relieve to some extent when he sill up or leans forward, and taking painkillers has helped him a lot. He stated that he got his pain while jogging in the early morning. On physical examination he shows, labs normal results, Elevated troponins, no hematomas observed on the chest wall, negative palpation for intercostal neuritis, rhythmic heart on auscultation, STAT EKG required, Cardiovascular service consultation, permanent monitoring of vital signs, patient has also stiffness and loss of motion of other joints. patient who presents symptoms that suggest myocardial infarction, laboratory confirms elevated cardiac enzymes, pain 9/10, EKG shows left lateral infarction.
