Problem
Pho Chan is a 103 yo. Chinese male is admitted to the med/surg floor with complaints of weakness, confusion, and multiple falls. He is accompanied by his adult daughter, Mayling and adult granddaughter, Sueling. Mr. Chan and his daughter speak Mandarin and have limited to no ability to speak and understand English. Sueling has been acting as the interpreter for the family. PMH: DM II, 67-year pack a day smoker, HTN, A-fib, chronic anticoagulation, CHF, AICD (automatic implanted cardiac defibrillator) Home Medications: Lopressor 100mg BID, warfarin 5mg daily, Lisinopril 10mg daily, Lasix 40 mg BID, atorvastatin 40mg qpm, tamsulosin 0.4mg qhs, restoril 15mg qhs, Seroquel 25mg qam & 50mg qhs VS: temp 36.1, RR 22, SPO2 90% on RA, HR 60, BP 62/38 Labs: Na+ 165, Cr 2.5, K+ 5.2; Hgb 6.8, WBC 9,000, Hct 21% Code status/advanced directives.
