His wife adds "Earlier we used to go out for a movie every weekend and visit our son once a month but for the past few years all he wants to do is rest in bed". The patient says "it is all because of my past bad deeds and I know that I deserve to suffer". He also expresses a wish to be better off dead but denies any active suicidal plans. Past treatment records show trials on escitalopram, sertraline, mirtazapine, and venlafaxine. He is currently off medications for the past 2 months. Based on the symptom profile and past treatment records he is started on a new class of medication but strictly advised not to eat or drink foods rich in tyramine (aged cheese, red wine, meat) while taking this medication owing to the life-threatening side effects due to drug interactions with tyramine.
