A 30-year-old patient presents to the office with five days of left lower leg pain and two days of left foot/ankle weakness. He indicates that he was kicked in the front of his left lower leg two or three weeks ago while playing rugby. At the time he received the injury, he iced the area and refrained from exercise for a few days. When the pain diminished, he resumed his usual 35-40 minute daily run. However, after a few days of his usual runs, he found that the pain recurred with activity. Over the last couple days, he notes the pain has progressed to a dull pain that persists throughout the day, accompanied by a foot/ankle weakness. He has received minimal relief from ibuprofen (400 mg three times daily), ice packs, and muscle rubs. Physical examination reveals he is not in acute distress. His pulse is 65 beats/minute, blood pressure is 110/70 mmHg [Normal: 119/70 mmHg], height is 183 cm (72 in), weight is 75 kg (165 lbs), and body mass index is 22.4 kg/m2 [Normal: 18.5-24.9 kg/m2]. Examination of his left leg shows no deformity or tenderness to palpation of the anterolateral lower leg. Dorsalis pedis pulse is not palpable in the left foot. Muscle stretch reflexes in his knees and ankles are symmetrical. He has pain with attempted left foot dorsiflexion, which he is barely able to accomplish against gravity.
