Problem

The nurse is caring for a 92-year-old male client (he/him) in a urology unit. Below is a copy of a nursing note about this client: 1100h: Client was seen in the emergency department complaining of lower abdominal discomfort rated 4/10 on a Numerical Rating Scale, found to be incontinent of urine in the hallway of the long-term care home. He was trying to find the bathroom in his room and became disorientated. Client presents with a past medical history of: coronary artery disease with PCI to LAD in '95, benign prostate hyperplasia, COPD, left hip fracture due to frequent falls. Client accompanied by adult child. Vital signs: T: 38.3°C (100.9°F); BP: 143/86; P: 86; RR: 18; with pulse oximetry of 92% on room air. Client presents as pleasant, cooperative, alert, and orientated to person, place, and time. No confusion was noted while on urology, but he was placed in a room close to the nurses' station. Gait steady, denies any dizziness, but requires one person assist for support only. S1 and S2 auscultated with S3, apical heart rate regular and equals radial pulse.
