Question: Respond to In treating insomnia in a 72-year-old female with depression, two pharmacologic agents that could be considered are trazodone and mirtazapine. Both medications can have synergy to address depression and insomnia with sertraline (Stahl, 2021). Both medicines can counteract any potential sleep disturbances caused by sertraline, allowing for better sleep quality and promoting a better response to depression. Trazodone in older adults should have lower starting doses due to increased sensitivity to sedative effects and risks of orthostatic hypotension (Shin & Saadabadi, 2022). It works by blocking serotonin 2A receptors in the brain and inhibiting serotonin reuptake. A blockade of the alpha-adrenergic 1 receptor can cause dizziness. Trazodone has a relatively short half-life, which may benefit older adults at higher risk for morning sedation (Stahl, 2021). Additionally, trazodone has minimal anticholinergic effects, which can be advantageous in elderly patients who are more prone to experiencing cognitive side effects from medications with anticholinergic properties
