Sean presents as a 34 y/o African American male seeking treatment for cocaine use disorder severe and cannabis use disorder severe. Sean grew up an urban part of Detroit where resources were limited and with a single parent. Sean has 5 siblings and being the oldest he had to get a job at as a young teenager to help his mother. He unfortunately got involved with selling drugs, gangs, and the street life. He never really saw anything wrong with what he was doing, especially since he made more money than when he worked a regular 9-5 job. He was exposed to substances from a young age and would often smoke marijuana with minimal consequences. When a trying time occurred he used crack cocaine and never looked back. He often states he's fearful of letting his brothers (fellow gang members) know that he became addicted to the substance that he sells.

• In group, Sean is insightful but with talking about others. He presents as extremely intelligent and has become a natural leader of the group. Sean struggles with personal reflection in session. When challenged in group or session he can identify what he needs to...but something seems lacking with connection and application to himself with what he verbalizes. There seems to be a disconnect there.

Where would you start the treatment planning process with Sean? What cultural considerations would you have to take into consideration? Sean is planning on returning home after treatment. What should you prep him for?
