Case study
Diane is a 22-year-old single woman who makes an appointment to see a psychology professional because she is having difficulty with her dating relationships. She recently lost her job and reports feeling "down" all of the time. Diane stated that when she feels "down" she would sometimes make cuts on her abdomen, as she enjoys the "release it provides." She denied needing medical attention for any of the cuts. Diane very dramatically tells you all of the details about her relationship with her last boyfriend and at the end of her story begins crying wildly. When asked for some information about her family and friends, Diane tells you that her family has disowned her and she does not have any friends. She goes on to tell you that she is desperate and must have your help immediately. When asked about the use of any substances, Diane becomes defensive and says, "Of course I have a drink occasionally.....doesn't everybody!". She goes on to note that she is drinking on a daily basis, which includes an increasing amount of both vodka and beer over the past 3 weeks. You note that throughout the session, Diane seems to idealize and then devalue you as a clinician. She states that, "I think you may be the only person who will be able to help me out of all of the therapists I've ever seen". Then, Diane mentions that, "It doesn't seem like you really know what you're talking about do you?".
