PMH: Hypothyroidism-diagnosed 3 years ago Medications: ELTROXIN 50 µG tablet po daily Allergies: none OBS/GYN History: G0P0, Cycle 28 days x 5 days, menarche 13 yo. Last pap smear four years ago and normal. Sexual Hx: 5 lifetime partners- male, no barrier use; Bacterial Vaginosis 2 years ago. No current vaginal symptoms. Lifestyle/Psycho-social: Lives with her partner, David, who is a criminal lawyer Tobacco: Social smoker Alcohol: Socially Occupation: Angela is a psychologist. Note: History gathering/screening tools are not considered a diagnostic test. At six weeks post-partum Angela comes to see you, and during the visit she suddenly bursts into tears. She reports that her partner has been working long hours and is 'never at home' and that she is struggling to keep up with the 'duties' of parenthood on her own. At the end of the visit Angela asks about the need to continue her Eltroxin as she stopped taking it just prior to giving birth because she had run out of her medication.
