Assignment: Prepare for an Intake
Instructions
As Human Services workers, we get referrals with new client information all the time. Often these referrals come with a short description of the client and why they need our services. As we prepare for that first session, we either reflect on ourselves or with our supervisor or team about the information we know.
In this assignment, choose 1 of the 3 clients described in this document below:
Scenarios
Joseph is an 80-year-old man residing in his own home. Joseph has 3 children who all live over an hour away. Joseph was widowed 3 years ago, and he has struggled to continue to engage in activities that he would do with his wife. His wife had done all the cooking, scheduling and organizing of the household. Joseph's children all work and have families, resulting in them being busy and one child checking in each week. The children had noticed that Joseph is not keeping up with cleaning and has lost weight. He seems to not be keeping details straight and is presenting as forgetful. Joseph misses going to mass on Sunday and hosting big family dinners like the family held before his wife died and their children got so busy with their own family. Joseph is saddened things are different than when he was raising a family and they prioritized spending time with grandparents. Joseph's children contacted you because they want to see him enter the senior living community where you work. They reported that he recently fell but would not use the life alert system or call one of them for help. This was the last straw for the children worried about their father's safety.
Emma is an 8-year-old young lady who lives with her father and stepmother. Emma has been struggling at school for the past quarter. Emma has always done very well in school but recently her grades began to drop. Emma made friends with the new girl, Beth, about the time that her grades began to be an issue. Emma reports that she doesn't want to be a nerd and only nerds do their homework. Emma's stepmother has been in her life since she was a baby but recently Emma has been very rude and disrespectful to her stepmother. She has been asking lots of questions about her birth mother who left her when she was 6 months. Emma's dad reports she doesn't want their help as much as she used to, and she wants to do things herself with little assistance. You are the Parent Liaison and received information from Emma's teacher about this issues that are presenting following the parent teacher conference where she learned about the issues at home. Emma's teacher reports that Emma's interactions have changed since she and Beth became friends. Emma's teacher tells you (but cannot tell Emma's parents) that Beth moved to your school because of issues with her mom and she is now living with her grandmother and is a very angry young girl.
Jordan is 18-years-old. He lives with his parents and is a senior in high school. Jordan has been more and more interested in activities that do not involve his parents. He works at a coffee shop near the local college and has begun spending more and more time with friends from work rather than his friends from high school. Jordan is looking at colleges and keeps debating leaving his hometown or staying and going to school with his new friends. When Jordan is home, he prefers to spend time in his room and rarely talks to his parents. Jordan's parents are concerned about this behavior and have brought him in to your office because they are worried that he is depressed or has a mental health issue because of all the changes. When you meet with Jordan, he engages easily, reports on his goals for college and ideas he has about his future career. Jordan reports he enjoys writing and that his friends from work have invited him to the college authors' club. Jordan states that he appreciates what his parents have done to help him but he is wanting to "do his own thing" and he is looking forward to next year when he starts college.
Prepare 1-2 page document to share with your supervisor to help you prepare for your intake. You need to use the scenario to assess what aspects of development are on track using the developmental milestones, what issues or concerns are present using the developmental milestones, and identify variables present that are impacting development. You should identify theories that apply to help understand the client's functioning. In referral information, we often get a brief summary, so it is important to be prepared with questions you need to ask to better understand the case.
