Case Study: Max is 85 years old. He has lived an active life but his health is now failing as his dementia progresses. He has seen many friends die recently. Some of them were not prepared and Max saw the confusion and stress this caused their family. Max decides to prepare an Advanced Care Directive in case something happens to him and he is unable to explain his wishes.

Question: What should the health care team do if Max has a stroke and becomes incapacitated and is unable to make decisions about his care? Max's condition worsens; he now has severe confusion and is showing signs of disorientation and erratic behaviour. What would you do in this situation? Max is having difficulty swallowing and often chokes on any liquids. Due to this he is refusing to eat or drink. What should you do about this? . Using the care plan below what changes could be suggested to encourage him to eat/drink so that his hydration and nutritional needs are met?
