For Discussion, you are required to view the documentary about Big Mama who raises her grandson, Walter who was in foster care. While watching this video which is 39 minutes in length, I want you to do the following:
1. View the video and take notes about Walter and Big Mama
2. Assessment Phase: Write up an assessment with Walter and his grandmother that incorporates and reflects the situation and his needs as presented in the interview. Please review the Assessment Chapter in the Woodside Book to include components of the Assessment Phase. You are to write a couple paragraphs for this section.
3. Case Planning Phase: If you were Walter's Case manager, what plan of action would you recommend. Please be sure to base your plan with the best interests of Walter in mind and all that we have read for the class. Think critically. Are the interventions in the video the best for Walter? Why or Why not? Please discuss the Care plan in detail for about two paragraphs and address the questions that I have posed about the interventions.
4. Case Implementation: What plan of actions would you carry out or implement as Walter's case manager? What steps? What resources would you need to recommend? What type of networking would you do on Walter and Big Mama's behalf ? What case management roles would you take on in this case example? Broker? Coordinator? Advocate? Again this section should be about two paragraphs.
5. For this post you are also required to answer the following:
Write a summary reflection of what you have learned this semester about case management and its importance to the social work profession. provide a thoughful essay on what you have learned in this class that you think will benefit you the most in your social work career. The answer to this question should be at least 2/3 of a page response. You can include anything that you have learned either from a guest speaker, activities in class, lectures, interviews with case managers etc. to answer the post. What did you enjoy the most? the least?
