Based on the literature review completed, identify the solution that you determined would be most effective in resolving the stated problem {stated problem: Hospice nursing provides a compassionate, holistic, end-of-life care that for patients while giving them dignity, comfort, and quality of life. However, providing the idealistic form of care can be challenged by other system-related and individual barriers. One major issue in hospice care is the disparity between palliative care's theoretical principles and how consistently and effectively these principles are applied in practice. Factors that can contribute to disparity, includes variations among nurses' knowledge about the practice of palliative care, the emotional exhaustion that staff may experience from providing palliative care, and the logistical challenges with integrating the philosophy of hospice care into standard nursing practice protocols. Evidence-based interventions can help to resolve these challenges in hospice.} and define the intended outcomes of implementing the change (e.g., your improvement goal). For quality to improve, a change must occur. That change must be quantifiable. In other words, it must be measurable.
I. Create an implementation plan in which you:
1. Explain how you will measure the change and how you will know when you have reached your improvement goal.
2. Create a list of outcomes required to reach your outcomes goal. This will allow you to determine the actions needed and the priority of tasks that will result in the desired outcome.
