Desmond, age 9, lives at home with his mother who is chronically ill with lupus and unable to transport him to a clinic for therapy. His mother also struggles with understanding Desmond's emotional needs and with organizing stable routines in the home. Desmond helps care for his mother, including translating for her when they go to stores. She has a home health aide who speaks her language for medical needs and a cousin who helps with housework. Desmond worries about his mother's health and has frequent nightmares. Desmond is socially isolated, except at school, and his 3rd grade teacher notes that he spends most of recess playing alone and sometimes cries in class when he is called on.
