Case: 40-year old-male with progressing fever and cough with hemoptysis A 40-year-old male presents to your office with nasal congestion, mild sore throat, productive cough, and low-grade fever with malaise for 4 days. Today his fever has increased to 101 F, and he noticed "specks of blood" in his sputum. He teaches at the local high school, is married with two teenage children Has a 10 pack-year history of cigarettes. His past medical history is negative. Medications reported are NSAIDs for occasional muscle ache. On examination Looks ill with continuing cough and chills. T 101.2 F BP 140/82 Pulse 110 Respiratory rate 20 Oxygen saturation 95% on room air. There is mild erythema of the mucosa of the nose and posterior oropharynx. Ronchi are auscultated in the lungs, with decreased breath sounds. There is dullness to percussion in bilateral lung bases.
