Problem

Jimmy is an 8-year-old boy who is brought to the clinic complaining of a persistent dry cough for the past 2 months. His mother reports that Jimmy had what appeared to be a cold with a sore throat and a low-grade fever 2 months ago. Jimmy says he recovered from his cold and sore throat after about a week or so but has not stopped coughing. Jimmy has one of his paroxysmal cough episodes while in the office. You witness him coughing hard and hear a loud sound when he finally is able to stop coughing and inhales. Jimmy has received no childhood immunization - his parents are anti-vaxxers. Jimmy's temperature, respiration, and heart rate are within normal limits; his chest X-ray shows clear, healthy lungs; and he does not appear in any distress. The attending physician explains that infants born prematurely and patients with underlying cardiac, pulmonary, neuromuscular, or neurologic disease are at high risk for complications of from this microorganism (e.g., pneumonia, seizures, encephalopathy or death).
