Questions: Conducting a focused literature review and incorporating a theoretical framework are essential when it comes to developing evidence-based practice. To address this quality improvement project, certain steps need to be followed to ensure focusing our resources on beneficial literature. General steps for conducting a literature review include:

1. Identify the research question: We need to clearly identify and phrase our focused research question. A possible research question in our case would be "What interventions or practices have been effective in decreasing hospital admissions for adult patients with heart failure?"
2. Set search criteria: To ensure the most relevant search results, we need to establish criteria for our literature search. This can be done by specifying the publication date for up-to-date information, literature keywords for relevance, research design for desired methods, and the specific patient population in interest.
3. Collect data: An efficient data collection strategy is essential. We need to gather relevant information such as the study methodology, the implemented interventions, the reported outcomes, and the final findings of a study.
4. Analyze data: We need to examine the quality of the findings from our literature review by evaluating the relevance and accordance with our research question, as well as examine the validity of the findings to ensure we have extracted the best evidence that's available for our project's purposes.
5. Summarize results: We need to summarize and organize our review findings from the literature review. It's important to identify the problems that were in question, the tested interventions, and the outcomes for each study. This step is crucial if not the most important for developing evidence-based practice.
