You are a case worker within a hospital. You receive a referral from the emergency department (ED). Leanne, an Indigenous woman in her mid 30s has presented to the emergency room with a severe infection in her hand. The referral tells you that Leanne has repeatedly experienced the same infection, and that it has led her to the ED every time. Leanne identifies that the wound is related to her injection drug use. Leanne identifies that she struggles with ongoing chronic pain and has not received any preventative medical care in over 10 years. She reports that she sleeps on friends' couches when available, and that she has many friends who hang out at their "regular spot" to panhandle. The medical staff in the ED refer Leanne to your services as she identified a desire to change and "get on the right track". They inform you that if she continues to infect her hand, that she may suffer permanent nerve or tissue damage. In your first few minutes meeting with Leanne in her hospital room, she is very talkative and appears to be in great spirits. She tells you that she wants to "get out of this place". However, when you ask her to go into more detail about her relationships with family and friends, she appears to shut down and stops talking.
