Mrs Prue Wilson is a 34-year-old woman who is five days post-partum with her first child. You have been called to her home by her husband, who is concerned about his wife. Mrs Wilson delivered a baby girl vaginally at full term at the local hospital, without complications. The baby's APGAR score at five minutes was 8/10 and she weighed 3,875g. However, Mrs Wilson struggled to initiate breast feeding and became very worried about the baby's nourishment. Mr Wilson reports that his wife has not been sleeping much, partly because the baby disturbs her, but she also stayed up last night when the baby was asleep. Mrs Wilson seems vague when you talk to her about her sleep pattern, stating that she can't recall when she slept and when she didn't. She becomes defensive and irritable and says, "I'm a good mother! I'm doing okay". She seems agitated and restless, and her thinking seems disconnected.

Mr Wilson reported that his wife has been behaving oddly today. He found her on the floor, looking under the couch and when he asked her what she'd lost, she replied, "I've lost my soul". She appeared very perplexed and somewhat agitated, according to Mr Wilson. He reported also that she had been inspecting the baby closely and asked him if this was really their baby. Early this morning, Mr Wilson found his wife standing at the crib staring blankly at the baby while the baby cried loudly to be fed.
